D&L ACADEMY REGISTRATION FORM

TERMS AND CONDITIONS

Private lessons:

1. Lessons are charged for the full term in advance
2. Full payment is required within 2 weeks of invoice date.
3. If invoices are outstanding, players will not be able to continue with any sessions until full payment has been received. (A total freeze on all sessions will be immediate and during this period all sessions will still be fully charged until invoices are settled).
4. Should a player need to cancel a lesson, at least 24 hours notice is required by emailing proacademy@livingtennis.co.uk and finance@livingtennis.co.uk and we will arrange for the lesson to be rescheduled. If a lesson cannot be rescheduled, we will issue a credit towards the following term.
5. If the coach is going to be away or cancels, we will provide cover with another coach, if no cover can be provided then a credit will be given for the following term.
6. Should a player not attend a lesson and/or the required notice has not been given, lessons will still be charged and the lesson will not be rescheduled.
7. If notice can be given at the start of the term for lessons that cannot be attended (i.e exam clashes, school trips) we will arrange for the lessons to be covered by another coach or rescheduled.
8. Long Term Sick or Long Term Injured:

- Should a player be off sick or injured short term (3 weeks or less), their lesson/s will be kept open for the player and parents will still be charged for these weeks.
- Should a player be sick or injured for longer than 3 weeks, written notice must be given to proacademy@livingtennis.co.uk and finance@livingtennis.co.uk. A decision will be made between the player, parent and the team whether the player is to be removed from the program until well enough to return. The player will then be recorded as ‘Long-Term Sick’ or ‘Long Term Injured’ and parents will not be charged from the 4th week for the remaining weeks of term until such time that the player can return to lessons. All Full Time players’ and RPDC players’ to refer to their player contracts.

Squads: (day time and after school)
1. Squads are charged for the full term in advance.
2. Full payment is required within 2 weeks of invoice date.
3. If invoices are outstanding, players will not be able to continue with any sessions until full payment has been received. (A total freeze on all sessions will be immediate and during this period all sessions will still be fully charged until invoices are settled).
4. No make up sessions are given for missed sessions, we can try to accommodate players attending on another day if space allows, please email requests to Tom at tgreenland@livingtennis.co.uk copying in proacademy@livingtennis.co.uk and Merle at mbramwell@livingtennis.co.uk
5. INJURY - in exceptional cases whereby the player cannot play for a number of weeks, we will require notification in writing, copying in our finance department, finance@livingtennis.co.uk so we can "freeze" the dates/credit the account.
